St Vincent's Health and Community Services has a full time opportunity for an experienced Clinician to join our Aged Care Assessment Team (ACAT) based in Bathurst. This role is available on a maximum term contract until the 18th of April 2020 working 38 hours each week.

About the role:

In this role, you will join the ACAT team and will be responsible to undertake comprehensive assessments and determine the overall care needs of individuals and assist them with access to the most appropriate types and level of care services that best meet their needs. Your duties will also include:

  • Promoting accurate and progressive legal documentation of care;
  • Liaising with departments within respective services regarding matters associated with the operation of ACAT;
  • Participating in quality improvement initiatives, and more.

To be considered a suitable candidate, you will possess the following:

  • Current registration with the Australian Health Practitioner Regulation Agency as a Registered Nurse or Allied Health Professional;
  • Post graduate qualifications in an Aged Care related clinical specialty;
  • Current NSW Drivers License and willingness to travel as a course of employment;
  • A minimum of 3 years RN experience in community care services, residential care and/or healthcare or Allied Health Professional Grade 2;
  • Current NSW Health Immunisation Status - Category A;
  • Excellent oral and written, interpersonal and liaison skills;
  • Understanding of and commitment to Quality Improvement.

Please note, the successful candidate will be required to complete a Certificate 3 in Aged Care Assessing.
