Lourdes Hospital and Community Health Service (Lourdes) is a third schedule, not-for-profit teaching hospital funded by NSW Ministry of Health and Managed by Catholic Healthcare. Lourdes has a long history of providing excellent care to the Dubbo and greater region. An opportunity currently exists for an experienced Case Manager to join our dedicated Brain Injury Rehabilitation team on a permanent part time basis.

About the role:

You will be responsible for the case management of Traumatic or Acquired Brain Injury clients. Assessing client rehabilitation needs and setting client goals through direct contact with the client as well as liaising with all other relevant personnel. You will need to be able to develop and implement individual goal focused rehabilitation plans, and advocate for clients to ensure best possible rehabilitation outcomes. Responsible for promoting and maintaining professional practice and an efficient team environment at all times.

To be successful for the role, you will have:

  • Demonstrated ability to plan, implement, and evaluate client focused rehabilitation plans
  • Demonstrated ability to work collaboratively with clients, carer's, therapists and local services within regional and remote communities
  • Demonstrated knowledge about community-based rehabilitation and the case management role
  • Knowledge of current issues pertaining to care of people with a Traumatic Brain Injury
  • Experience in establishing rapport and communicating with colleagues, clients and their families effectively in a non-judgmental manner
  • Evidence of flexible work practices that support a dynamic work environment in servicing a diverse and complex client group.
  • Eligible to apply as an approved case manager with Lifetime Care and Support
  • Current NSW Drivers License
  • Degree in an allied health or registered nursing
  • Eligible for registration/membership with APHRA or the appropriate professional supervisory body
